Self-screen for COVID-19 symptoms

Students, faculty, and staff should self-screen for COVID-19 symptoms prior to coming to school each day.  The screening should include taking your own temperature and answering the following questions:

  1. In the past two (2) weeks have you had close contact or direct contact or live with someone who has been diagnosed with COVID-19?  If the answer is “yes” then you need to stay home.
  2. In the past two (2) days have you had COVID-19 symptoms such as feeling feverish or measured temperature greater than or equal to 100.4 degrees Fahrenheit, loss of smell or taste, cough, difficulty breathing, shortness of breath, headache, chills, sore throat, shaking or exaggerated shivering, significant muscle pain or diarrhea?  

If the answer is “yes” then you need to stay home. If you answered yes to any of these questions and are required to stay home, please contact your school nurse for further instructions.
